Its a feature not a bug.
Dominion can change the scanner settings to allow a majority of ballots to be considered "errors" so they get sent to digital adjudication and then be adjudicated for "voter intent".

Some counties “adjudicated” 94% of ballots. The only true audit is to review all ballot images and to see what votes were changed.
